According User Manual for manually add node I should use: "... click Add Node on the Tools menu." But there is no Add Node in 1.2.3 (both nxmc and web). I try to use the most similar "Find Ip address..." with IP 172.16.100.100 (pingable, not in node list). I get Warning dialog "Connection point information cannot be found." (both nxmc and web). And in server log is:

But after while node appears. Is that wanted behavior? And what about adding nodes before they are configured (set IP etc.), is it possible?

To manually add nodes, right click on the "All Services" object in the Object Browser, which should be on the left-hand side of the window (If not, press F3, or look under the View menu to open it). Then select "Create node..." from the dropdown menu. You can also create Containers this way to organize your nodes.
The node probably appears automatically after a while because network discovery is enabled. You can find its options under the Configuration menu.
The user manual is unfortunately a bit outdated, the wiki contains some more up to date information but isn't quite complete yet either: http://wiki.netxms.org
